Hello and Welcome to our Forum! Many people contact us to ask how do they obtain NHS Continuing Care Funding or even start the process. I've listed below the options available to get you on the road to the first step and the rest of the information can be found in the attached Guide. 1) Ask your GP or District nurse to undertake a Checklist for who ever requires the assessment; This is the first part of the assessment process. 2) Your relative will then be graded, A, B or C as to how severe their care needs are in areas such as mobility, cognition, continence etc. 3) If they score highly enough on the Checklist, this doesn't mean they will be entitled to the funding. It simply means that they will then be eligible for the full assessment, known as the Decision Support Tool. Take a look at the attached guide for further information.
